First, I am sure if you have ever driven stick and downshifted without "rev matching" you feel the car jult forward when you let the clutch out in the lower gear. This is because the lower gear causes the engine to spin at a higher RPM and if you don't feed in more gas during the shift the engine will jump up to the higher RPM all at once. Normally when you are downshifting (and NOT braking) you can "blip" the throttle with your right foot in the middle of the shift. But what about when you are braking? How would you get your right foot to the throttle without taking it off the brake? This is where heel and toe comes in...
Heel - toe downshift: say you are driving hard and late braking for a turn. well you might be in third gear and want to come out of the turn in second for proper acceleration. if you simply put the car in second and let the clutch out you will throw off the balance of the car...no good (a result of the "jult" i previously talked about). So while you are in the middle of shifting from 3rd to 2nd, you shift your right foot so that you "blip" the throttle (raising the RPM's about 1500-3000 RPM's depending on your car) while still keeping constant brake pressure. This allows the engine to match the new speed it will be at when you let the clutch out in a lower gear.
Usually you keep your toes on the brake and use the right side of your heel to push the gas. On BMW's it is very easy because the gas pedal is floor mounted allowing easy change over from brake to gas.
One more reason that BMW's are the ultimate driving machine

really, its toe on brake, heel on gas? that could be why i suck at it. iv been doing heel on brakem toe on gas. i tend to put too much on the brake...

I agree, I can't get my heel on the gas well enough so I do more of a big toe/ball of foot on the brake and the right side of my foot on the gas. Usually either do too little or way too much on the gas though... still working the balance out.
i can do it, just that i have to think about it alot, it's not nearly as quick and natural as i'd like it to be, i'm getting driving shoes soon with a reinforced heel toe area on the right shoe, not that i need them, but i bet it will make driving a lot more fun

I got some low top puma driving shoes off of the puma website about a year ago, those are great, I do not think that the right toe has anything speacial about it though....nope it dont I just checked, but I would strongly recommend them. They are a bit pricey but they are also the only par of "tennis shoes" that I have so I end up wearing them about everyday and besides being dirty they hold up well.
