scanning readings

I have just used a scanning tool and it displayed the following codes:

#61 - Evap Small Leak
#A6 - Vanos Output Stage Cyl #5-8
#7d - Radiator Outlet Temperature Sensor

Can anyone help me understand what is wrong with the car?

The EVAP code pops up when you either have a vacuum leak or the gas cap is loose.

The VANOS one should like a leak in the VANOS system. Is your engine making any odd noises?

The last one sounds like the temp sensor in the radiator is bad.

I have been having problems when first starting the car, like the automatic choke is faulty, the car strutters for quite some time, then eventually after the car has warmed up (about 5 mins) the car runs fine.
