Clunking Trasmission

recently Ive noticed a clunking noise when my car automatically shifts gears at lower rpm's

Is this the start to a serious problem?

It is a single clunk noise

If im driving slow the noise isnt so bad because the trans shifts smoother

but if im driving a little quicker it is a loud clunk
its very noticable

A related problem
If my car is parked uphill when Ishift into reverse (automatic) it is very dificult and when it does shift I hear a slighter version of the clunk noise.

Is all the weight of you car on the transmission when you are parked on a hill? because that can be really bad for your trany after a while. Or do you have the ebrake on with no weight on the trany?

Sorry about the delay the car has been parked on a medium grade hill for about 4 months since i bought the car
