The a/c is a closed system. If you had to get it refilled, then chances are you have a leak. Not ever using the a/c can cause the seals to dry out. There is a dye you can add to the a/c system to locate the source of a leak.
You don't have to get converted to r134a I wouldn't immagine, but your wallet may thank you. You have to have a license to buy R12 and it isn't cheap, but you can go to Discount Auto and get a can of r134a. Note though that converting from R12 to r134a includes changing the fittings, the refrigerant, and the oil. R12 oil and r134a do not play nice together.
The clunk you hear might be the compressor engaging.
